Understanding Your Home Care Needs

Before we provide home care services, it’s essential to fully understand the care needs, wishes, and feelings of the individual. For family members arranging care, we take the time to listen to your needs and answer any questions you may have, ensuring you feel reassured that your loved one is receiving the right home care. Our home care process is designed to provide personalised, compassionate support to each individual’s needs, ensuring peace of mind and comfort throughout the care journey.

While each individual’s situation is unique, we aim to follow these procedures after receiving an initial enquiry:

Contact within 24 Hours

We will reach out to arrange a home visit within 24 hours of your request or your family’s request.

Preliminary Home Visit

A member of our trained office team will visit your home to assess your personal needs. They will review your current situation, evaluate what care you may require based on your capabilities, and discuss your options with you and your family.

Developing a Care Plan

We will collaborate with you to create a personalised care plan. This plan will specify the number of visits required and the type of care to be provided during each visit. We will also agree on visit times and inform you about who will be visiting you.

Approval and Costs

Care will only begin once you and your family are satisfied with the care plan and fully aware of the costs. You will be asked to sign the care plan to confirm your approval, ensuring you feel in control and involved in your care.

Our goal is to assign a small, consistent team of carers to manage your care, fostering familiarity and building a strong rapport. This approach ensures that both you and the care team are confident and content with the care plan.

Our Comprehensive Home Care Process

1. Electronic Records:

Your individual notes and care plan are securely recorded on our Pass for Care App. This system allows your care team to update your notes daily and document any changes, ensuring everyone stays informed about your progress. We encourage you and your family to access Pass to stay involved and aware of any updates. Additionally, we invite you to share any concerns or request changes to your care plan, which can be promptly addressed.

2. Regular Evaluations:

Our care manager and office team will regularly review your care plan with your care team. Formal evaluations occur every six months, or as needed based on changes in your condition. These evaluations are also opportunities to ensure your satisfaction with your care team and make any necessary adjustments.

3. Progress Updates:

Between evaluation periods, we are always available to provide progress updates to you or your family. These updates can be arranged flexibly or on a scheduled basis, according to your preference.
